Home Health Aide

a trained and certified care worker who provides assistance to a patient in the home with personal care (as hygiene and exercise) and light household duties (as meal preparation) and who provides health care under the supervision of an RN Case Manager.

Skilled Medical Care – nurses

Skilled nursing care by a RN or LPN/LVN either on an intermittent or hourly basis including:
• Controlling infections
• Pediatric nursing
• Performing dressing changes
• Palliative care
• Wound care
• Insulin therapy
• Pain management
• Ventilator and tracheotomy care
• Care for chronic illnesses and injuries
• Brain and spinal cord injury care
• Orthopedic care
• Diabetes management and care
• Post surgery care
• Implementing a clinical care plan
• Coordinating home medical equipment, pharmacy, and supplies
• Monitoring vitals

Intermittent Care

Intermittent care enables the patient to receive the highest quality healthcare only at the time that assistance is needed, eliminating the cost of full-time, around-the-clock care. Whether you are recovering from surgery, illness, or managing a disease, PHCS is here to provide nursing care, rehabilitation, and support services in the comfort of your home.

Therapy

Our therapists customize a rehabilitation program around the individual needs of each patient. Our staff includes Physical Therapists, Occupational Therapists, and Speech Language Pathologists. Evaluations, treatment, and education are provided for individuals with total joint replacements, orthopedic conditions, neurological disorders, post-surgical procedures, stroke rehabilitation, and speech difficulties.

Respite Care

Respite Care Services enable individuals to comfortably remain in their own environments, while providing caregivers with a much needed interval of rest or relief. A period of respite may be a few hours to a few days in length.  These short-term breaks can help to relieve stress, restore energy, and promote balance in your life.  It also enables you to share the responsibility for care-giving while getting the needed support for yourself and loved ones.

Our Respite Care Services include, but are not limited to*:
• Assistance with activities of daily living
• Performing personal care (bathing, grooming, etc)
• Dressing
• Meal preparation
• Performing light housekeeping
• Laundry
• Entertaining
• Oral hygiene and skin care
• Incontinence care
• Assistance with ambulation
• Obtaining pulse, temperature, and respiration

Pediatric Care

PHCS realizes that there are few greater joys than to see children enjoy their lives fully playing, smiling and engaging with their families. That is why we have dedicated ourselves to caring for children with special needs and providing support for their families. At Pro Health Care Services, we know that no two children are the same. That’s why our pediatric services are designed on a case-by-case basis, specific to the needs of the child. We believe that offering quality care to children is not only important, it’s essential.

PHCS Pediatric Services include, but are not limited to the following*:

• Performing personal care (bathing, grooming, etc)
• Tracheotomy care
• Enteral feeding/tube feeding
• Ventilator care
• Dressing
• Preparing meals and feeding
• Oral hygiene and skin care
• Assisting with mobility
• Incontinence care
• Obtaining pulse, temperature, and respiration
• Implementing plans of care
• Attending to disabilities, chronic illness, and/or therapies

• Administering prescribed medications and/or therapies
